Abstract

Nurse-led discharge planning programs play a crucial role in ensuring smooth transitions for patients from hospital to home care. This review article aims to assess the effectiveness of these programs in improving patient outcomes and reducing hospital readmissions. A comprehensive literature search was conducted to identify relevant studies on nurse-led discharge planning interventions. The findings suggest that nurse-led discharge planning programs have a positive impact on patient satisfaction, adherence to post-discharge instructions, and overall health outcomes. These programs help in facilitating communication between healthcare providers, patients, and their families, leading to better coordination of care and improved patient experience. Moreover, nurse-led discharge planning programs have been shown to reduce healthcare costs by minimizing unnecessary readmissions and emergency room visits. However, challenges such as limited resources, time constraints, and variability in program implementation exist, affecting the overall effectiveness of these programs. Future research should focus on standardizing discharge planning protocols, evaluating long-term outcomes, and addressing barriers to program sustainability.
