Abstract

Interprofessional care transitions play a crucial role in ensuring seamless healthcare delivery and improving patient outcomes. This review article aims to evaluate the effectiveness of interprofessional care transitions in reducing hospital readmissions and enhancing continuity of care. The significance of effective care transitions cannot be overstated, as they are essential for promoting patient safety, reducing healthcare costs, and enhancing overall quality of care. The review will examine various studies and interventions that have focused on interprofessional care transitions and their impact on reducing hospital readmissions.
